Registration Flow For Streamyx

1. This section is only for customer who does have a telephone line and wish to apply for Streamyx only. For customer who does not have a telepone line and wish to apply telephone line + Streamyx, kindly proceed to TELEPHONE section.

2. Partners have to download and print out the application form from Tagter.com as printed copy is accepted by TM. However, you may request for original copy from your manager if you want to but the quantity is limited.

3. Different types of application forms are for different packages:
  • Business Broadband Application Form - This is for normal SOHO and Fixed IP Package
  • Business Broadband Office In A Box Application Form - This is for OIAB Package
  • Streamyx application form for Blockbuster Deals with TM homeline - This is for Blockbuster Deal Package
4. After filling up all information, partners are required to create a new customer order in Tagter.com and upload all supporting documents.

5. Supporting documents required for personal package:
  • Application form with customer signature (Printed copy or original copy)
  • A clear photocopy IC of customer for Malaysian or a clear photocopy passport of customer for foreigner (Passport expiry date must not be less than 2 years from registration date)
6. Supporting documents required for business package:

 Sole Proprietor (ROB)
  • Application Form with Company Stamp
  • SSM ROB - Form D or Form E
  • NRIC / Passport
 Sdn Bhd (ROC)
  • Application Form with Company Stamp
  • SSM ROC - Forms 9, 24 & 49
  • NRIC/Passport
 Berhad (ROC)
  • Application Form with Company Stamp
  • Form 8 and 23
  • Authorized Person Form / Letter and NRIC/Passport
 Foreign Company
  • Application Form with Company Stamp
  • Form 79, 80/80A, 83/83A
  • Authorized Person Form / Letter and NRIC/Passport
 Law Firm
  • Application Form with Company Stamp
  • Bar Council Malaysia Acknowledgement Letter
  • NRIC/Passport
 Clinic
  • Application Form with Company Stamp
  • Letter from Malaysia Medical Council and Form 12
  • NRIC/Passport
 NGO/Others
  • Application Form with Company Stamp
  • Acknowledgement Letter from the Related Ministry/Department
  • NRIC/Passport

7. We will process your order only if we receive all supporting documents.

8. Your order will be rejected if you fail to provide all supporting documents.

9. Once your order has been processed into TM portal, order status will be updated. If you do not understand the status, kindly refer to STATUS REMARK or contact your manager.

STREAMYX INSTALLATION

1. Installation will be done within 1 week after registration if your order was processed successfully. Contractor will contact your customer for installation.

2. Installation appointment date and contractor's detail can be found via http://sams.tm.net.my/VendorCenter/search.asp. You may call up the contractor to push for installation or rearrange appointment date.

3. Problematic order such as order with "Kaji Siasat" or "Tunggu" status takes longer time for installation. It may take more than 1 week, 1 month or even few months for TM to solve the problem.

4. There will be additional charges if your customer requests for extra cabling whereby customer has to deal personally with contractor.
